Primary Care & Community Nursing Framework

The Strategic Programme for Primary Care (SPPC) is proud to announce the formal launch of the Primary Care and Community Nursing Framework

The Primary Care and Community Nursing Framework has been developed by primary care and community nurses and outlines how they will contribute to achieving the ambitions of A Healthier Wales and the Primary Care Model for Wales.

The framework consists of:

  • A Vision for Primary Care and Community Nursing.
  • A National Framework explaining who we are, why we are critical and what is required to build a future model benefiting the people we see, nurses, other professionals and the health and social care system.
  • An action plan outlining how Primary Care and Community Nursing will achieve these ambitions.


The framework is for primary care and community nurses and highlights to Health Boards and Welsh Government, the contribution that primary and community care nurses offer towards “promotive, protective, preventive, curative, rehabilitative and palliative services for people, throughout the life course" (World Health Organisation, 2023, Primary Care).
