Swansea Bay UHB

There are 8 cluster networks with the Swansea Bay University Health Board area and the populations vary from around 30,000 to over 75,000.  There is clear leadership within the Clusters from GPs in particular and they have been developed on a multi professional basis. They now provide a structured mechanism for organising and delivering many community health and social care services, and have developed strong links with the third sector and other key partners.  The Health Board's Primary and Community Services Unit, is one of six Operational Units covering all of the clinical services provided by the Health Board, which leads on this agenda through it's Cluster Development Team. https://sbuhb.nhs.wales/community-services/gps-dentists-etc/clusters-working-together-for-a-healthier-you/
