Inclusion Health

The Wales Inclusion Health Programme for Primary Care webpages have been developed to support primacy care services developing or reviewing services to consider and adapt to the needs of their most vulnerable populations. 

Inclusion health can include any population group at the sharp edge of health inequalities. These groups experience stigmatisation, social exclusion, discrimination and experience “severe, overlapping and multiple disadvantages” across their lives, significantly increasing their risk of poor health. This has also been described as the ‘cliff edge of inequity’. 

Health inclusion is a matter of social justice and human rights but there are also benefits to society and to the individual. Although the building blocks for health sit outside of health services, we can be AWARE of them and how they impact on some of us in our communities and build AWARENESS in others. We can ADVOCATE for the building blocks in our communities and the role in health. We can also take ACTION to move towards a universal equitable primary care system in Wales.
We are working with people across Wales on a ‘Health Inclusion’ programme and have developed tools and resources to support you in your work in your community whether in awareness, advocacy or action.

  • How we can make small changes to build trusting relationships in frontline services
  • As Team leaders or managers we can work with other local services to support each other on health inclusion
  • Senior or strategic leaders we can provide leadership and support health inclusion services
