The planning portal for cluster working, is a resource for those working within Primary and Community Care in Wales to support the planning and delivery of health and care services.
Planning and delivery of health and social care services in Wales should align to the strategic direction of the Well-being of Future Generations Act (Wales) 2015, in particular A Heathier Wales well-being goal and the sustainable development principles / five ways of working.
This resource provides an overview of the planning processes in Wales and the key elements to consider. Including considerations/ methodologies for assessment and response to local needs, linking to sources of further information, and supporting skill development.
The planning portal builds on the Cluster Planning Support Portal and Population Health Information by Topic toolkits.
