This page hosts summaries of projects that have been implemented by clusters across Wales. The list is presented by Health Board and by cluster. The summaries have been written and provided directly by Health Boards and contain relevant local contact details if you wish to access further information about any specific project. Please use this page to identify any similar projects you are considering, and lessons learned from elsewhere.
If you would like to submit a summary of a local cluster project, complete this templateand return to PrimaryCare.One@wales.nhs.uk. Please title the email subject as ‘Cluster Projects Repository - [Project Title]’. Summaries will only be uploaded on the page if they are provided in both Welsh and English.
