In Wales too many lives are being cut short due to unequal access to the building blocks of health - good education, safe homes, enough money and healthcare. Health inequalities remain a challenge for Wales.
Healthcare is one of the building blocks of health. Primary Care plays a vital role in reducing health inequalities as the most accessible point of contact within the health system addressing both medical and social needs. Although the drivers of health inequalities sit outside the health service, how they are delivered can reduce or increase health inequalities.
Primary care works in and with communities and is the right place for personalised prevention, early intervention and treatment.
It is important that we work to together to improve the ways in which the primary care system in Wales can tackle health inequalities.
Although the causes of health inequalities are complex there are simple actions for each of us, within our control, which together can make a difference.
These page aim to
• Support colleagues working in, or with primary care to consider health inequalities across service planning, delivery and evaluation
• Build capacity, knowledge and understanding of how to reduce health inequalities in primary care
Included within these pages are Teg I Bawb – Fair for All Strategic Action Plan and other documents which aim to support primary care systems to take action to reduce health inequalities and support their most vulnerable populations. This work has two main areas of focus.
Reducing Health Inequalities in Primary Care Programme
Including the Teg I Bawb- Fair for All Strategic Action Plan and Health Equity toolkits. Building on feedback this programme aims to help the primary care system to identify actions it can take which can reduce health inequalities.
Inclusion Health Programme
Aims to build awareness, capability and knowledge of how the primary care system can support populations facing extreme and multiple disadvantage.
