Primary & Community Care (PCC) Metrics Dashboard

The Primary and Community Care (PCC) Metrics Dashboard is a new national resource designed to improve how Wales understands and uses data about the care delivered in communities every day. Primary and community care provides the majority of patient contacts across the NHS, yet the scale and impact of this work has often been difficult to see. This dashboard changes that by bringing key information together in one place, offering a clearer picture of activity, demand, and outcomes across Wales. 

Developed in partnership with Health Boards, local authorities, independent contractors, and national teams, the dashboard builds on the PC Metrics work undertaken in 2023–24. It brings together existing datasets, standardises how information is presented, and provides a high‑level overview of PCC performance at cluster, Health Board, and all‑Wales levels. Users can also access links to more detailed dashboards, making it easier to explore specific areas of interest without duplicating existing analytical work. 

The dashboard is designed for anyone who needs timely, reliable insights into primary and community care—whether for planning, performance discussions, ministerial briefings, or service improvement. By increasing visibility of PCC activity, it supports more informed conversations across Welsh Government, NHS Wales Performance and Improvement, Health Boards, and contracted providers, helping identify opportunities to improve services, reduce bottlenecks, and enhance patient experience. 

The PCC Metrics Dashboard represents a significant step toward a more transparent, consistent, and connected approach to understanding primary and community care in Wales—supporting better decisions, better conversations, and ultimately, better outcomes for the people and communities we serve. 

Key information  

Alignment with Existing Work 

The dashboard does not replace or duplicate existing detailed dashboards. Instead, it provides a high-level overview and links directly to more granular resources. 

Use of Existing National Data 

Leveraging already available datasets and information in the public domain ensures efficiency, reduces duplication, and supports a comprehensive and relevant dashboard. 

Gateway Function 

The dashboard acts as a gateway to other PCC related dashboards. Information governance arrangements for those dashboards remain unchanged. 

System Overview Display 

An agreed minimum dataset will provide a concise, systemwide view of the most important PCC metrics. 

Iterative Refinement 

Updates will be made at a regular interval (likely biannually), informed by user feedback and changes in data availability. All updates will be communicated to stakeholders via email.

Access
