Primary and Community Care Compendium of Roles and Models website

The Primary and Community Care Compendium of Roles and Models website has been produced on behalf of the Workforce and Organisational Development workstream of the Strategic Programme for Primary Care (SPPC) in collaboration with Health Education and Improvement Wales (HEIW).

Background

It builds on the original concept of the Compendium of Emerging Roles and Models in Primary Care launched by WEDS (Workforce Education and Development Services, now part of HEIW) in 2018.

As primary and community care roles and service models continue to evolve, we have revised the content and functionality, bringing you a re-designed, up to date resource which will expand going forward.

The website Home | Primary Community Care Compendium (heiw.wales) is now LIVE, ready for access, reference, and promoting with colleagues. 

What is it

The website was developed and populated with twelve case studies last year in phase one, then soft launched for feedback on the look and feel of it. In phase two supplemental content was added to existing case studies and new case studies were developed in order to grow the compendium further.

Each case study spotlights those delivering care models currently ‘live’ in Wales which showcase integrated multi-professional working to provide person centred care, closer to home. Those involved were professionals with lived experience, working at the top of their licence, in new or innovative, with links on each page to related resources, such as regulatory bodies and careers information.

How can it be used

Share good practice by supporting essential research and development activities, before commissioning services to meet local population needs, by reviewing tried and tested examples.

As a once for Wales platform for the sharing of innovative and unique models of primary and community care provision it saves time, money, and effort involved in local promotion; enabling case studies to be utilised, via hyperlink, to support aligned work and avoid duplication.

New case studies are welcomed and can be submitted via the Submit a Case Study button on the Homepage and Case Studies page. A form and instructions on the process are provided.
