The Eastern Vale Cluster is in the process of developing its plan for 2024-25. It is based on the local population needs assessment, plus the emerging priorities identified by members of the cluster.
They are developing projects to support its local population.
Eastern Vale has the second highest percentage of elderly patients, and is striving to keep patients well at home instead of spending periods of time in hospital. To achieve this, they have an enhanced care at home programme plus a multi-disciplinary team working together to support this cohort of patients. In addition, the cluster has a Frailty Nurse who undertakes comprehensive reviews, falls assessments, future care planning and admission avoidance. There is also a housebound influenza service to provide timely vaccines for housebound patients. A Cluster Pharmacist has been employed to provide additional capacity to meet the medication needs of patients.
They are working on new initiatives to address some of the cluster emerging priorities. These include pre-diabetes interventions, lifestyle choices and mental health.
