There are seven practices in the Cardiff West Cluster area

With the new Cluster Structure/Network established we are continuing to develop relationships and learning across the professional collaboratives, to support better care and patient education, as well as establishing new integrated ways of working and developing projects that all the professional collaboratives can contribute to.
The Cluster has invested in providing additional Pharmacist resource, currently providing services within the General Medical Services Practices – we are continuing to look at new ways of working and how this resource can be developed further, with ambitions of developing a Multidisciplinary Team / Hub Model.
We have also been working on introducing the UPCC model of care to the Cluster, providing an additional 160 same day appointments to deal with acute conditions every week.
Further to this, the Cluster is exploring Social Prescribing, and how we can better connect patients to activities, groups and services within the community to meet practical, social and emotional needs that affect their health and wellbeing.
The Cardiff West Cluster has established new ways of working, in line with Accelerated Cluster Development (ACD), bringing together the Primary Care Collaboratives to work collectively for the first time.
This includes: General Medical Services, Dental, Optometry, Community Pharmacy, Community Nursing, Allied Health Professionals, and the Third Sector.
The Cluster has agreed new Terms of Reference and governance and implemented appropriate administrative resources to effectively support Cluster / Collaborative working which will enable us to carry forward projects that achieve collective goals and aim to improve patient care and outcomes for our Cluster Population.
Within the Cardiff West Cluster, we have a growing aging population, some of our future priorities to support our population needs include Diabetes Prevention, Falls Prevention and Chronic Wound Care.
Additionally, we are also looking at Childhood Immunisations and exploring innovative ways in which we can positively contribute to both uptake and education.
Another key priority within our Cluster is the Local Development Plans within our footprint, we will continue to explore new ways of working to meet the increasing demand on Primary Care services.
Further to this, we are also exploring ways we can support staff wellbeing to create a more resilient and effective workforce that will support primary care sustainability and improve outcomes and processes for our patients.
