Individual Cluster Plans and Reports

North Denbighshire Cluster IMTP 2020-2023

What are we working on?

Due to the resignation of the cluster lead and inability to find a replacement, North Denbighshire were unable to spend any of the funds for 2015/16.  We have been successful in the appointment of 2 cluster leads and progress has been made on funding schemes for this year 2016/17

What we have already done

The cluster have focused on improving life for patients suffering from chronic pain by working in collaboration with Pain Association Scotland, setting up an intensive self-management course consisting of local monthly group meetings within the cluster. The group provides better understanding of chronic pain and finding positive ways to manage health. This directly supports the patients to take ownership of their conditions whilst providing capacity in practice for GPs to see more complex patients. An evaluation of this service will be carried out, early feedback from patients is positive.
