BCUHB Cluster Population Health Directory - The North Wales Population Health Directory (NWPHD) focuses on the public health priorities and provides information on population health need at different geographical levels (including at Arfon Cluster level) to NHS staff for planning purposes. Arfon Cluster IMTP 2020-2023
Cluster plans are created after liaising with the practices within the Cluster and following a review of the Practice development plans. They are subsequently amalgamated by the Cluster lead, Cluster co-ordinator and Senior Cluster Co-ordinator, to create the Cluster plan. The same team create the annual report.
