BCUHB Cluster Population Health Directory - The North Wales Population Health Directory (NWPHD) focuses on the public health priorities and provides information on population health need at different geographical levels (including at Arfon Cluster level) to NHS staff for planning purposes. Individual Cluster Plans and Reports

Arfon Cluster IMTP 2020-2023
Cluster plans are created after liaising with the practices within the Cluster and following a review of the Practice development plans.  They are subsequently amalgamated by the Cluster lead, Cluster co-ordinator and Senior Cluster Co-ordinator, to create the Cluster plan.  The same team create the annual report.

What are we working on?
  • We are working on creating a GP COTE post, to employ a GP to review our care home patients and discuss treatment escalation plans with them.
  • We are supporting the Cluster in attempting to improve the uptake of the flu vaccine.
  • We are supporting a near patient testing-CRP pilot
  • We are supporting a faecal calprotectin pilot project, giving local GP’s access to the test in primary care.
What we have already done

We are currently supporting the following projects:

  • Advanced physiotherapist in practices
  • Clinical pharmacist in practices
  • Supplemented phlebotomy sessions in primary care
  • Arfon Social Prescriber/ Community link officer for third sector services
  • Supplementing the community diabetic dietician post to develop community diabetic services
  • Supporting training and education of primary care staff
