South West Cardiff Network Cluster Action Plan 2017-20 

What are we working on?

We are continuing to develop our Integrated Care Hub, expanding the team to include a multitude of professions to establish new integrated and innovative ways of working. The team currently includes Care Navigators, Pharmacists, a Health Care Support Worker, an Occupational Therapist and a Project Manager – who work collectively to improve and align services to meet the needs of our population.

After the success of our Childhood Immunisation project, we are expanding this work to improve Immunisation engagement and education for our teenage population.

Further to this we are currently engaging with Local Schools to explore what services can be developed to improve the health and wellbeing of students based on what matters to them.

Accelerated Cluster Development has evolved the way we work as a cluster, we are continuing to build relationships and share learning across the professional collaboratives to collectively implement projects and services that will improve the health and wellbeing of our population.

What we have already done

The Southwest Cluster have implemented an award-winning Quality Improvement Project that have improved engagement and awareness around childhood immunisation for our population through coordinating local events and engaging with the community.

We have a well-established Multidisciplinary Team Model that incudes our Integrated Care Hub, which ensures safes medicines management and supports patients to remain safely at home following discharge from hospital.

Further to this, we have developed excellent Social Prescribing services through partnership with ACE to better connect patients to activities, groups and services within the community to meet their practical, social and emotional needs.

Additionally, we have invested in BAME counselling services through Community Care and Wellbeing Service (CCAWS) to support our diverse cluster population.

Future Priorities for the Cluster

We are committed to the continual review and development of our current projects and services, with the aim of adapting and expanding successful projects to further meet the needs of our population.

Further to this, we are interested in exploring potential ways the UPCC model could be introduced to the cluster to provide additional support for GMS sustainability through the provision of additional appointments for our patients.

Diabetes Prevention, Dental Advice and Education, and Cancer Patient Pathways are some further priorities the Cluster are looking to explore in the future.
