What are we working on?

The cluster is currently working on a Diabetes Prevention initiative in collaboration with the All-Wales Diabetes Prevention Programme – training Health Care Assistants within Practices to empower patients at risk of developing type 2 diabetes to proactively manage their lifestyle to minimise the risk of developing the condition, which can have a significant impact on health and wellbeing.

The cluster is committed to the continual development of its staff, as such, we have been supporting cluster staff to complete their Independent Prescribing training, with the aim of creating a more skilled workforce and better access to services for patients.

We are also currently exploring ways in which we can collaborate with Secondary Care to improve waiting times for patients and ensure patients are seen by the right person first time. 

What we have already done

As part of our commitment to develop new ways of working, we have introduced regular Multi-Disciplinary Team meetings and established a ‘Wellbeing Hub’ within the cluster, both of which look to support joined up and integrated ways of working that deliver more efficient and effective patient care – with a focus on keeping patients safe at home.

Additionally, we have been working on improving our cluster populations’ health and wellbeing through Social Prescribing by working with ACE to better connect patients to activities, groups and services within the community to meet their practical, social and emotional needs.

Further to this, the cluster have invested in providing additional resource to carry out ‘home visit’ appointments to our most vulnerable and frail patients, through the employment of an Advanced Paramedic Practitioner. This additional resource has improved access to general medical services for housebound patients and released capacity to enable GPs to see more patients and deal with complex cases within their Practice.

Similarly, the cluster has provided additional support for patients with respiratory issues such as COPD and Asthma through the employment of a Specialist Respiratory Nurse who has been able to provide specialist appointments, improving diagnosis and management of respiratory conditions.

As a cluster we have also established an Urgent Primary Care Centre (UPCC) within the cluster, which provides an additional 240 same day appointments every week for patients who are experiencing acute conditions. 

Future Priorities for the Cluster

In line with Accelerated Cluster Development (ACD) the Cardiff North Cluster model has evolved to include each of the professional collaboratives: General Medical Services, Dental, Optometry, Community Pharmacy, Community Nursing, Allied Health Professionals, and the Third Sector.

As we move forward, we will continue to develop our collaborative approach, and explore new integrated ways of working, to collectively improve the health and wellbeing of the cluster population whilst supporting primary care sustainability.
