The North Wales Population Health Directory (NWPHD) focuses on the public health priorities and provides information on population health need at different geographical levels (including at South Wrexham Cluster level) to NHS staff for planning purposes. What are we working on?

Development and Implementation of the role of Social prescribers and community agents as part of a wider signposting project.

Development of new SLAs with Medicines Management and Therapy Services for Advanced Practitioners.

Cross Cluster Projects - Mental Health event in October 2018. 

What we have already done

Wrexham South Cluster Action Plan 2017-2020 redacted version  

Signposting work, developed setting up and developing existing websites with signposting information and links to the DEWIS database, training practice staff in order to navigate patients to the correct service/health professional.

Worked with Wrexham County Borough Council to deliver six steps training to care home staff within the cluster to support residents to be supported at home at End of Life.

Practices ran OOH flu clinics during the winter in order to increase the number of patients vaccinated.

Cluster physiotherapists and pharmacists employed.

Led on Information Governance work in relation to Cluster Data Sharing 

Future Priorities for the Cluster

Agreeing SLAs and contracts for service delivery.

Social Prescribing and wider signposting activity.

Continuation of cross cover for the two single handed GPs within the cluster
