GP Practices in Caerphilly East
There are 3 Neighbourhood Care Networks (NCNs) in Caerphilly. There are seven practices which operate in the Caerphilly East Cluster area.
Lloyd Hambridge has been working within the Caerphilly East NCN since 2015 as a NCN Pharmacist, as an Advanced Practice Prescribing Pharmacist for the Blackwood Medical Group of surgeries within the NCN and have recently completed the Welsh Clinical Leadership Fellowship programme with HEIW and Welsh Government. Lloyd has clinical interests which include respiratory disease, “green” prescribing and medicines management processes focusing on improving patient safety.

As part of the wider Advance Care Planning work, Glan yr Avon Residential Care & Nursing Home has just completed the Improving Quality Together (IQT) Silver Award.
Caerphilly East NCN has awarded funding to Communities First to assist in the provision of X-POD, a 6 week health prevention and education program. The sessions are aimed at diagnosed pre-diabetic patients and those who want to reduce their risk of developing Type 2 Diabetes.
Communities First are also delivering Foodwise, an 8 week program developed to help individuals manage their weight the healthy way. The sessions, some of which are delivered at some GP practices, have been developed by Public Health Dieticians in Wales and linked with the Welsh Government Change 4 Life Campaign
