There are multiple models of social prescribing working in Wales with practitioner roles based in places such as local authorities, third sector, housing, higher education, and primary care; and social prescribing activities most commonly based within third sector and community spaces. The aim of this work is to understand the landscape of social prescribing activity across Wales, and where possible, to quantify and describe its functions. |

In doing so these toolkits were designed to assist Health Boards to: • Review their current bedded community facilities; • Look at where they could apply this resource in regard to D2RA pathways 2 and 3; • Optimise use being made of the facilities for patients; and • Consider the flow of patients through these complex systems to the best effect. The toolkits were “road tested within three Health Boards, a summary of the feedback from those mini pilots, has been included in this publication. |
