Cluster Self Reflection

Cluster self-reflection is one of three agreed methods for evaluating the progress of cluster working towards the Primary Care Model for Wales (PCMW) and Accelerated Cluster Development (ACD). The self-reflection exercise aims to:

·       Identify system factors that hinder or facilitate cluster working.

Primary Medical Care Advisory Team (PMCAT) - report into the review

Enhanced Community Care 

The paper by the Strategic Programme for Primary Care provides the Once for Wales agreed definition, core standards and underpinning quality statements, of an Enhanced Community Care (formerly virtual ward) model of care and support. Providing a base from which community based services can deliver high-quality evidence-based care to people and populations requiring enhanced community care. 

This updated paper outlines the ECC Evaluation Framework which includes a set of measures and outcomes. This enables services to demonstrate the value and effectiveness of their ECC model of care and support in a consistent way across Wales. Ensuring people receive safe person-centred care regardless of the population the ECC focuses on, and an opportunity to share learning from this way of working in a consistent way across Wales.

ACD Supporting Governance (Model paper for health board use)

This paper has been written in order to provide a framework for Health Boards to use, such that they are able to clearly articulate the relationship between the Health Board, newly forming Pan Cluster Planning Groups and Clusters. This will allow the Health Board in due course to be confident in exercising the functions of planning ,commissioning and delegation / approval of resources.

This paper sets out the key governance principles, overarching legislation and suggested governance for consideration by health boards in developing and implementing the planning and delivery mechanisms described in the Accelerated Cluster Development programme.

Primary Community Care Escalation Framework. Revised  2022

Building on the original Escalation Framework released in December 2021, this revision is more streamlined with a focus on impact and actions as well as triggers and levels. The framework aligns with the Welsh Ambulance Service Operational Delivery Unit Escalation Report used in the daily joint Welsh Government and NHS Wales safety huddles.
The Action Card Compendium is a collection of established primary and community service actions cards brought together in a central compendium for those seeking to create new action cards or to benchmark existing ones against.

Understanding social prescribing in Wales: a mixed methods study (final report)

There are multiple models of social prescribing working in Wales with practitioner roles based in places such as local authorities, third sector, housing, higher education, and primary care; and social prescribing activities most commonly based within third sector and community spaces. The aim of this work is to understand the landscape of social prescribing activity across Wales, and where possible, to quantify and describe its functions.

Bedded Community Facilities Mini Test Outputs v1

The Delivery Unit was commissioned by the Strategic Programme for Primary Care 24/7 work stream to review bedded community facilities. In doing so these toolkits were designed to assist Health Boards to:

• Review their current bedded community facilities;

• Look at where they could apply this resource in regard to D2RA pathways 2 and 3;

• Optimise use being made of the facilities for patients; and

• Consider the flow of patients through these complex systems to the best effect.

The toolkits were “road tested within three Health Boards, a summary of the feedback from those mini pilots, has been included in this publication.
