Aneurin Bevan Health Board Implementation of Transformation Model within Primary Care The aim of the scheme is to support GPs within managed practices by transforming the way that care is delivered, thus sustaining Primary Care services and release capacity for GPs to focus on acute appointments in their Practices and achieve better patient satisfaction. 
Aneurin Bevan Health Board Implementation of General Practice Nurse training placements within Primary Care The aim is to develop a nursing workforce within General practice in appropriate numbers and with sufficient skill mix to improve the health and wellbeing of the local population and prevent avoidable health problems, whilst also meeting any planned or unplanned health needs that the population may have.

Community Treatment Centre To reduce workload in general practice and address ongoing sustainability by transferring workload away from general practice and secondary care outpatient departments into community based treatment centres staffed by specialised practitioners (e.g. vascular and TVN nurses, / injectable teams, podiatrist technicians).

Betsi Cadwaladr Health Board Advanced Practice Paramedics – Urgent Home Visiting Service The proposal is for a 24/7, 365 days model of Advanced Practice Paramedics, providing a rapid response service to patients requiring home visits, which would previously have been provided by their GP.

Cardiff & Vale Health Board Expansion of Medicines Management Resource – Pain clinics utilising independent prescriber qualifications The aim of the project is to trial and develop a pharmacist led clinic, in primary care, to review pain medication. 
Cardiff & Vale Health Board Social Prescribing Pathway – To develop a model of social prescribing across Cardiff and Vale UHB A model of social prescribing is proposed which is based on a three tier system: Tier 1 Signposting, Tier 2. Social prescribing, Tier 3 Behavioural intervention.
Cardiff & Vale Health Board Collaborative Community Falls Pathway – To reduce the incidence of falls within Cardiff and Vale.  The key aim of the project is to test the approach used in Canterbury within the Cardiff and Vale area, working across partner organisations within Cardiff and Vale to develop and deliver a cohesive community response pathway/model to reduce the incidence of falls.

Development of Advanced Training Practice Hubs for Preregistration and Advance Nurse Practitioner roles This Pacesetter is focusing on the development of a sustainable nurse workforce at both the pre-registration stage and also the Advanced Nurse Practitioner. The existing practice nurses needs are already supported by the Primary Care Support Unit by way of mentorship, coaching and also training programmes.
