Development of the Health Equity Action Framework for Primary Care in Wales

Recognising the unique role of Primary Care in addressing health inequalities, the Primary Care Division of Public Health Wales is in process of developing an Action Framework aimed at addressing health inequalities through primary care.

Our intention is for this action framework to be grounded in evidence, driven by data, and collaboratively created with key stakeholders.

After conducting a literature review, three in-person stakeholder workshops took place in October 2024 to collaboratively identify key action areas.

The dialogue continues with additional online workshops involving key stakeholders and service users scheduled for early 2025, with the goal of producing an action framework accompanied by a strategic action plan by April 2025.

Clinical interventions to reduce health inequalities within primary care. this report outlines the findings of a search conducted by the Public Health Wales Evidence Service to identify evidence on primary, secondary, and tertiary prevention interventions aimed at reducing health inequalities within primary care.
